Becoming a Provider

A provider is an agency or individual that applies and is approved for certification with the Ohio Department of Developmental Disabilities (DODD). You can become an Independent Provider, Agency Provider, or a Direct Support Professional (DSP).

Please note that DODD is the only certifying entity for all Agency and Independent provider applicants of Home and Community Based Services (HCBS), Supported Living, Individual Options (IO), Level 1 (LV1), and SelfEmpowered Life Funding (SELF) Waivers. The Putnam County Board of DD can provide support to applicants for this process, but has no authority to certify providers.

Are you interested in becoming a provider?

If you or your agency would like to apply to become a Provider of Waiver and Non-Waiver Services, you must be certified through DODD. The applications process to become a certified provider is completed online at DODD.

Support for Providers

The Putnam County Board of DD is dedicated to supporting providers. Training Classes and other supports are available through collaboration with partnering agencies.
